Prior to your visit, we will do our best to verify insurance coverage and obtain authorization for treatment; however, we recommend you follow up to confirm the specifics of your insurance policy. A parent or legal guardian must accompany anyone under age 18 for their appointment.
Please take a look at what you will need to bring with you to your first visit and complete these forms before coming in for your appointment. This will allow our therapists to spend time with you during this initial visit listening and observing in order to design a custom treatment plan for you.
What to Bring to Your First Appointment:
- Patient Information Forms
- Pain/Injury Assessment (Select the one that is appropriate for your condition)
- Lower Back Pain
- Neck Pain
- Upper Extremity Injury Form (pain or difficulty in head, neck, arms, mid or upper back )
- Lower Extremity Injury Form (pain or difficulty in low back to feet)
- Patient Specific Functional Scale
- A written referral or consult form from your referring physician, podiatrist, dentist, or other provider
- Insurance card and photo identification
- A list of medications you are currently taking
- Co-pay and deductible payments (cash, check or credit card)
- Wear comfortable clothing
